Our state of the art production facility is complemented by a highly experienced civilian supervisory staff ready to serve your vision hardware needs.

PROVIDER ELIGIBILTIY

New customers can submit a complete Provider Information Form to determine eligibility (note: agency policy may affect provider eligibility). Please contact CI Optical Customer Care for any questions.

ORDER INSTRUCTIONS

Eligible eye care service providers can send their completed Health Care Authority (HCA) qualified order and/or frame kit to our CI Optical Customer Care team. Please note, CI Optical does not sell or credit frames for patients to try on.

Fill in and submit a complete Provider Frame Order Form.
Provide a legible medical prescription (Rx) with complete information (i.e., eye size, DBL (skull or cable temples), and any applicable codes).

Jobs start when patient’s frame arrives with the completed CI Optical order form. Do not fax patient's own frame (POF) orders.
